Interpretation: Significant pyuria (WBC 60-80/HPF) + bacteriuria — UTI confirmed. Send for culture and sensitivity (CSST). Dysmorphic RBCs + granular casts — glomerular involvement (CKD-related). Heavy proteinuria (3+). Recommend: ACR on early morning urine. Treat UTI empirically with Nitrofurantoin 100 mg BD × 5 days pending C&S (adjust for renal function).
