I am referring Mr. Suresh Patel, 52-year-old male (UHID: UH-2026-0814), currently admitted in the CCU under my care, for further Cardiology management and follow-up.

Presenting Complaint:
Mr. Patel presented with acute onset central chest pain radiating to the jaw on 21st June 2026. ECG showed ST-depression in V4–V6 with dynamic T-wave changes. Troponin was markedly elevated at 4.8 ng/mL.

Hospital Course:
He was diagnosed with NSTEMI and underwent urgent coronary angiography on 22nd June 2026. Angiogram demonstrated a significant (90%) stenosis in the mid-RCA. PCI was performed with drug-eluting stent (DES) placement with TIMI-3 flow achieved. Post-procedure course has been uneventful.

Current Medications:
- Aspirin 75mg OD
- Ticagrelor 90mg BD (DAPT × 12 months)
- Atorvastatin 80mg OD
- Ramipril 2.5mg OD
- Bisoprolol 2.5mg OD
- Pantoprazole 40mg OD

Investigations (recent):
- Post-PCI Troponin: falling (0.02 ng/mL)
- Echo (pre-discharge): LVEF 50%, mild hypokinesia inferior wall
- eGFR: 68 mL/min — monitoring for contrast nephropathy

I would be grateful for your review for ongoing cardiac rehabilitation, optimisation of medical therapy, and long-term follow-up. Please advise regarding any further investigation (e.g., stress MPI) and the timing of repeat echocardiogram.
