Stroke and brain injury rehabilitation — physiotherapy, SALT, OT, and goal tracking

Ischaemic stroke — left MCA territory. Right hemiplegia + aphasia.

mRS 4 (moderately severe disability)
Rehab Goals:
- • Return to supervised ambulation (walker)
- • Swallowing rehabilitation (currently NG tube feeds)
- • Expressive aphasia — daily SALT 30 min
- • Pressure ulcer prevention
Team: Neurologist, Physiotherapist, Speech and Language Therapist (SALT), OT, Dietitian
Progress: Physiotherapy: actively moving right arm against gravity Day 17. Standing with 2-person assist. SALT: no safe oral intake yet — MBSS scheduled. Carer training ongoing.
Daily physiotherapy + OT + SALT. Target: safe oral intake before discharge. Inpatient rehabilitation continued. 4-weekly mRS reassessment. Community rehabilitation referral on discharge.

Traumatic brain injury — Road traffic accident, GCS nadir 9, contusion right frontal lobe

Rehab Goals:
- • Full ADL independence
- • Return to work (accountant)
- • Cognitive rehabilitation — executive function
Team: Neurologist, Physiotherapist, Neuropsychologist, OT
Progress: Day 3 post-injury. Oriented ×2. Memory impaired (Wechsler MQ estimated 72). Ambulatory with minimal support. Cognitive fatigue prominent.
Neuropsychology assessment complete. Graded cognitive rehabilitation program. Return to work unlikely < 6 months — support letter for employer. Follow-up outpatient neuro-rehab.
