Indication

Suspected pneumonia / endobronchial lesion query. BAL for culture. Baseline CT suspicious LLL opacity.

Technique

Olympus BF-H180 bronchoscope. Patient supine, monitored. Topical lignocaine 4% spray. Midazolam 2 mg IV + Fentanyl 50 mcg IV.

Findings

Vocal cords normal. Trachea midline, no lesion. Carina sharp. All lobar and segmental bronchi visualised. LLL posterior segment — mucosal erythema + purulent secretions. No endobronchial lesion. BAL performed at LLL.

Specimens

BAL (LLL) — sent for C&S, AFB smear, fungal stain, cell differential

Complications

None

Impression

Purulent secretions in LLL consistent with bacterial pneumonia. BAL sent. No endobronchial malignancy. Await culture results.
