Assess necessity — is CVC still required?
Inspect exit site — redness, swelling, discharge
Chlorhexidine impregnated dressing — change if soiled or loose
Check dressing date — change every 7 days (CHG-gel)
Check and document catheter length at skin
All lumen patency confirmed — flush with 10 mL NS
Document catheter blood-draw if required (use 5 mL discard)
Positive displacement caps changed (every 96h or after blood draw)
IV lines — check for kinks, secure all connections
