IPD-2026-00401 — Meera Iyer
Patient anxious but cooperative. Reassured — some improvement in anxiety. IV access patent bilaterally. Foley catheter draining clear urine at 40 mL/hour. Oxygen via face mask at 10 L/min — SpO₂ maintaining at 94%. All medications administered as per MAR. Patient NBM for planned PCI at 11:00. Family (husband) notified and present. ECG leads attached for continuous cardiac monitoring. Alarms set. No falls risk intervention required — patient on bedrest.
Patient received from ED at 06:10. Admission assessment done. Vitals recorded — BP 90/60, HR 110, SpO₂ 88%. O₂ commenced, IV access established. Loading doses of Aspirin and Clopidogrel given orally at 06:45. Heparin IV commenced 07:00, GTN 07:15. Cardiac monitor attached. Blood collected for stat investigations.
