Worsening creatinine (3.2 mg/dL), oliguria — query AKI superimposed on CKD. Guidance on fluid management and need for dialysis.
Reviewed patient. AKI stage 3 on CKD3. Recommend: restrict IVF to 500 mL/day, furosemide 40mg IV BD, monitor UO hourly. If UO <20 mL/hr over 4h, initiate dialysis. Nephrology to co-manage.
